Patient Forms

We are so excited to see you! Please fill out and submit the New Patient Form before your appointment. If you are a returning patient, please fill out and submit the Returning Patient Form. Also, please arrive to your examination 10 minutes before your scheduled time. Come prepared by bringing the following:

• Photo ID
• Insurance Cards (Medical and Vision)
• Medications List
• Prescription Glasses, Sunglasses, and/or Contact Lens Boxes
• A list of questions you want to ask your doctor so you don’t forget them
